Intensive Support Case Coordinator - FT, 12 Month Fixed Term Contract

As an Intensive Support Case Coordinator, you will play a key role in delivering our Intensive Support Programme, ensuring that people living with HIV who have a detectable viral load receive personalised, responsive support tailored to their individual needs.

Working closely with colleagues, healthcare professionals and partner organisations, you will coordinate a person-centred service that helps people overcome barriers, access the right support, and improve their wellbeing. Through effective partnership working, you will connect individuals with the services, opportunities and resources they need to thrive.
