National AIDS Trust (NAT) Coordinated Care Focus Group

 

As we get older, we are more likely to have health issues or need support, like social care, or support from services like George House Trust.

 

The aim of this focus group is to find out from you what help you might need to organise and coordinate all the different aspects of your care as they grow older. We are looking to speak to people aged 45 and over who have experience of using multiple health, social and well-being services.

 

We’ll hold the meeting on Zoom. There will be about 6 people on the call (all the participants will be registered with George House Trust), plus Josh from George House Trust and Cheryl from NAT. You don’t have to use your real name on the call if you don’t want to. We won’t use anybody’s name in the final report, all of our findings will be anonymized.

 

You don’t need to do any preparation for the meeting – we just want to hear about your experience of how well your care fits together. But, before the event we will send you some ideas about how care could be organised, just so you have an idea of the sorts of things we’ll talk about.

 

The Zoom will last about 90 minutes, and we’ll send you a £15 voucher to thank you for your help.

 

NAT is holding workshops with people living with HIV in Manchester and Newcastle. We’ll send you the findings of the workshops, and we’re also going to talk about them with HIV clinicians and other people who provide or organise care and support, so we can make the system work better.
